Requesting Your Health Records
You have the right to request access to your health records as permitted by law.
To request records:

Records are maintained in a secure electronic health record system (TherapyNotes). Requests are processed within the timeframe allowed by law. Fees permitted by law may apply. Psychotherapy notes are protected under HIPAA and may not be released except as allowed by law.
